Scout Name: Enter your scout's name WITH FIRST NAME and the FIRST LETTER of the LAST NAME. (Example: Ted L.)
Patrol: Select a patrol from the drop down menu
Cost: Most weekend events cost $30. Some events may have additional expenses.
Scout Phone: If you have filled out a yearly permission form you may simply enter "On File"
Parent Phone #1: If you have filled out a yearly permission form you may simply enter "On File"
Parent Phone #2: If you have filled out a yearly permission form you may simply enter "On File"
Parent Email: If you have filled out a yearly permission form you may simply enter "On File"
Emergency Contact: If you have filled out a yearly permission form you may simply enter "On File"
Emergency Phone: If you have filled out a yearly permission form you may simply enter "On File"
Allergy List: Medications, Food, Plants, Insect Bites. If you have submitted a yearly medical form you may simply enter "On File"
Medication List: List all medications currently used. Inhalers and EpiPen information must be included, even if they are for occasional or emergency use only. If you have submitted a yearly medical form you may simply enter "On File"
